O bjectives: Changes in hearing thresholds were determined
by standard and high-frequency audiometry in subjects
exposed to occupational noise for long durations.
Patients and Methods: Hearing thresholds were measured
between 250 and 16000 Hz frequencies by standard
and high frequency audiometry in 64 male workers
(mean age 42 years; range 31 to 55 years) of a hydroelectric
power plant and in 30 age- and sex-matched controls
with no hearing problems. The workers were divided
into three groups according to the noise level and into four
groups according to the duration of exposure.
Results: The mean hearing thresholds of workers
showed significant increases in all frequencies from
4000 to 16000 Hz (p<0.005), the most affected frequencies
being 4000, 6000, 14000 and 16000 Hz
(p<0.0005). Increases in hearing thresholds were
significantly correlated with the noise level and duration
of exposure to noise (p<0.05).
Conclusion: The results suggest that high frequency
audiometry should be used together with standard
audiometry in the detection and follow-up of individuals
who are at potential risks for hearing losses.

Objectives: We investigated the role of Borrelia
burgdorferi in the etiology of idiopathic acute peripheral
facial palsy.
Patients and Methods: Nineteen patients (15 females,
4 males; mean age 38 years; range 14 to 61 years)
with acute peripheral facial palsy were studied.
Following routine otolaryngologic examination, all the
patients underwent taste, Schirmer, and stapedial
reflex tests to evaluate the level of the palsy.
Laboratory examination included routine biochemistry
analysis, serum C-reactive protein, rheumatoid factor,
and erythrocyte sedimentation rate. Anti- B o r r e l i a
burgdorferi IgM and IgG antibodies were sought by
ELISA in venous blood samples.
Results: Of the patient group, acute facial palsy was
localized on the right in 12 patients (63.2%), and on the
left in seven patients (36.8%). None of the patients had
bilateral involvement. No IgM or IgG seropositivity for
Borrelia burgdorferi was detected in the serum samples.
Conclusion: In highly endemic areas, it may be helpful
to detect or even to eliminate Lyme disease through
screening of serum in patients with acute peripheral
palsy.

Objectives: We investigated the incidence of gastroesophageal
reflux by scintigraphy in patients with
laryngopharyngeal symptoms and findings.
Patients and Methods: The study included 32 patients
(14 males, 18 females; mean age 38 years; range 17 to
55 years) who presented with laryngopharyngeal complaints.
All patients underwent routine ENT and headneck
examinations and rigid endoscopy of the larynx to
determine laryngopharyngeal symptoms and findings.
The presence of gastroesophageal reflux was sought
by technetium 99m sulfur colloid scintigraphy.
Results: The most common symptom was dysphagia
(56%) followed by hoarseness (46%). Posterior
laryngitis was the most common laryngeal finding
(18 patients, 56%). Scintigraphy revealed gastroesophageal
reflux in 21 patients (66%), 14 of whom
(67%) had posterior laryngitis.
Conclusion: Patients with laryngopharyngeal
symptoms and findings should be investigated with
regard to gastroesophageal reflux. Scintigraphy may
be a method of choice in this respect.

A forty-one-year-old man presented with right-sided
progressive facial nerve paralysis of one year duration.
Computed tomography of the temporal bone
showed a soft tissue mass in relation with the tympanic
segment of the facial nerve with destructive
changes in the facial recess region. Magnetic resonance
images before and after gadolinium injection
revealed a contrast-enhancing mass lesion in the
tympanic cavity. The tumor was removed through a
transmastoid approach and a cable graft from the
greater auricular nerve was used to repair the defect.
Histopathologic diagnosis was facial nerve schwannoma.
At the end of one-year follow-up, the patient
had moderately severe facial nerve dysfunction.

Osteomas are the most frequent bone tumors of the
facial bones, the most involving the frontal sinus and
thus leading to symptoms of frontal sinusitis or associated
complications. A twenty-one-year-old male patient
presented with a growing swelling on the right side of
the forehead. Examination revealed a slight facial
asymmetry, a laterally shifted right orbital bulb, and disturbed
vision. Cranial and orbital computed tomography
showed a dense bony mass in the medial wall of
the right orbit. The mass was excised totally following a
bicoronal incision. The frontal and ethmoid sinuses
were filled with fat tissue. Postoperatively, his vision
improved to normal after two months and no complications
or recurrences were detected in the sixth month.

Lipoma of the deep lobe of the parotid gland is
extremely rare. A forty-two-year-old man presented
with a painless, soft, and slow-growing mass in the
left preauricular area. Ultrasonography and computed
tomography findings were consistent with lipoma.
The patient was treated with superficial parotidectomy
and total excision of the mass with preservation
of the facial nerve. Histologic findings confirmed the
diagnosis. No recurrences were detected during 1.5
years in the postoperative period.

Epithelial-myoepithelial carcinomas comprise approximately
1% of all salivary gland neoplasms. Most
tumors arise in the major salivary glands, especially in
the parotid gland. We present a case of epithelialmyoepithelial
carcinoma of the parotid gland in a 65-
year-old male patient. Magnetic resonance imaging
showed an irregular and heterogeneous mass in the
left parotid gland. Superficial parotidectomy was performed.
There was no evidence of recurrence during
a 21-month follow-up.
